That is not 100% certain. I've known plenty of projects that haven't used adequate safety precautions and have resulted in no deaths.

And how long did they last for?  Were they on the 'cutting edge'? Or were they more like the orion project, which sounded dangerous, but never detonated a single explosive?

Face it, if the project goes to completion without adequate safety procedures, and then keeps running, someone willl die eventually.

Nasa is afraid of (I believe) a 1 in 7000 chance that if the HST were deorbited it would hit something.  99% is nearly a sure chance.

If we're gonna go into space, of course someone will die eventually. Here's the best way for safe spaceflight: no spaceflight. Either the alarm won't sound or someone won't here it and get burnt to a cinder in a rocket launch, someone deorbits in the wrong way and gets incinerated, or something totally unexpected happens like to much fuel is used and th ocket gets stranded in the wrong orbit with air running out.

That is necessary risk.  I'm talking about unnecessary risk.  a few simple safety precautions can reduce risk to a little more than the reliability of your vehicle.

Oh, you sounded like you'd gone off topic onto the fact that someone will die eventually anyway if space travel becomes common.

Someone is going to die during a routine test of a rocket. If it's going on all the time.
