And how long did they last for? Were they on the 'cutting edge'? Or were they more like the orion project, which sounded dangerous, but never detonated a single explosive?
Face it, if the project goes to completion without adequate safety procedures, and then keeps running, someone willl die eventually.

If we're gonna go into space, of course someone will die eventually. Here's the best way for safe spaceflight: no spaceflight. Either the alarm won't sound or someone won't here it and get burnt to a cinder in a rocket launch, someone deorbits in the wrong way and gets incinerated, or something totally unexpected happens like to much fuel is used and th ocket gets stranded in the wrong orbit with air running out.
